School Clubs Charter Procedures and Forms

All school Clubs and Organizations MUST apply every school year to be an active club/organization for the current school year.  Complete the Application for School Club Charter form and Request to Establish a School Organization form and submit to Administration for approval.  
 
All clubs must have a faculty advisor, and have a club constitution on file.
 
You may email completed forms to [email protected] or drop off/mail to: Roosevelt High School,  1120 Nehoa St.  Honolulu, HI  96822.  ATTN: Student Activities Coordinator.
